Language differences
When I was doing my internship, there was an eastern European guy in my class. He had just immigrated, but had a decent grasp of English.

Occasionally, though, he'd encounter something that he only knew in the medical-speak of his home country. When that occurred he'd grasp at the closest-sounding English phrase he knew, and use that.

One night he was working, and I picked up his non-dispo'ed patients the next morning. One of the patients was a lady in her 60's.

In his note on her past GYN history, he had written "the patient began her climax* at age 51, and it has since continued."

*Menopause has had a lot of names over time. One of the old ones, not used as much today, is climacteric.
